**All players must fill out one of each form in order to be on the team. Note that your student ID number is located on your my.calpoly.edu page. Please fill out all forms completely and legible. You must also bring a photocopy of your:
1. Drivers License
2. Poly ID
3. Health Insurance
4. Drivers Insurance
5. CPR Certification (Only if you have one)**
