To Request Information From IP&A
If you have not found the information you are looking for in the publications and pages on our website, please follow the instructions below to submit a specific request for information from IP&A.
If you are on campus and have access to the my.calpoly.edu portal, please follow the SRS Ticket Instructions (PDF) to submit an SRS ticket (Technical Service Request) to IP&A. Once your Technical Service Request is submitted, IP&A will receive an automatic e-mail notification of your request.
If you are not on campus and do not have access to the my.calpoly.edu portal, please complete the IP&A Information Request (PDF) form.
