
Include Studnet ID number and date of birth on proof of immunization.
All new Cal Poly students, who are eighteen years of age or younger, are required to present proof of immunization against hepatitis B. The hepatitis B immunization is a series of three shots, usually given over a period of six months.
When Should Proof Be Submitted?
You will NOT be able to register for your second quarter until you have presented proof to Health Services that you have received at least one shot.
You must COMPLETE the series of immunizations prior to your second year of enrollment. You will NOT be able to register for your second year until you complete this requirement.
What Is Considered Proof?
A copy of any one of the following will satisfy your immunization requirements:
School Immunization Record
County Health Department Immunization Record (well baby booklet)
International Health & Immunization Record (used by military & foreign travelers)
Family physician's record
Verified record of blood test showing immunity to hepatitis B.
What If I Can't Get Proof?
Get immunized at Health Services. Or
Get immunized by your private health care provider or the Public Health Department, and send proof to Health Services.
